Common Issues With the Rotator Cuff

The muscles and tendons in your shoulder make up your rotator cuff . These muscles and tendons connect your humerus bone with your shoulder blade. They also hold the ball of your humerus bone in your shoulder socket. This union culminates to form the greatest range of motion of any joint in your body.

Common treatable issues include but are not limited to:

  • Tendinitis; an inflammation due to overuse or overload
  • Bursitis; the fluid filled bursa between the humerus and the joint can become inflamed
  • Strain or Tear
  • Normal wear and tear; occurs with age and overuse
  • Poor Posture
  • Repetitive stress; this occurs often in athletes or people in the building trade, such as painters or carpenters.

Treatments for rotator cuff issues include but are not limited to:

Non-Operative:

  • Exercises, stretching, postural changes
  • Physical therapy
  • Chiropractic
  • Injections
    • corticosteroids and/or local anesthetic
    • Subacromial bursa, AC joint, trigger points, carpal tunnel
    • AC joint, shoulder joint
    • Neck: foramental epidural steroids, facet injections (discograms)
  • NSAID’s
    • Celebrex, Mobic, Relafen, Lodine, Voltaren, Naprosyn, Motrin etc
  • Oral corticosteroids, eg..Prednisone, Medrol Dose Pack
  • Acupuncture
  • Supplements
  • Dynasplint for a frozen shoulder

Surgical:

  • Impingement
    • Acromiplasty vs bursectomy and subcromial smoothing
    • Smoothing / bursectomy 95% as good as acrmioplasty but fewer complications
  • Rotator cuff partial tears
    • Arthroscopic debridement, subcromial decompression, bursectomy
    • If >50% consider repair
    • Acromioplasty if cuff abraded from acromion
    • Subcromial smoothing is otherwise adesquate
  • Rotator Cuff Full Thickness Tears
    -Anthroscopic or mini open reapir

    • Treat other lesions
